Taking CAD to the classroom: The future of digital dentistry in the Middle East
Digital dentistry is the language of our time—and according to Dr. Rami Gamil, exocad is its alphabet. As CEO and founder of 3DVision in Egypt, Rami has been pioneering digital workflows since 2009. From guided surgery and smile design to education and entrepreneurship, he’s pushing dentistry to new heights in the Middle East and beyond. In this exoBlog interview, he shares his journey with exocad, the features he considers “superpowers,” and his vision for empowering clinicians through education and technology.
Q: Can you tell us a little about your journey into digital dentistry?
A: I graduated in 2006 and started exploring digital workflows in 2009 with CBCT for implant planning. That opened a new world for me. A few years later, I began experimenting with 3D printing for guides and models, which led to founding 3DVision—the first 3D printing and guided surgery center in Egypt.
My relationship with exocad started at IDS 2015, when I got my first educator dongle for DentalCAD. Since then, exocad has been central to my workflows, from waxups and bite splints to full-arch implant planning.
Q: What is the current state of digital dentistry education in Egypt and the Middle East?
A: There’s still a big gap—many universities only touch on digital dentistry superficially, leaving dentists curious but unprepared. That’s why we started 3DVision Education: a hands-on platform covering everything from scanning to CAD/CAM software like exocad, and manufacturing with 3D printing and milling.
Q: You’ll be speaking at the AOI Congress in Delhi. How does digital dentistry in India compare to Egypt?
A: I’ll present guided surgery workflows with exoplan, covering everything from scanning to surgical guides. India and Egypt both have passionate clinicians embracing digital tools, though India’s market is larger and more complex. The key challenge—and opportunity—is expanding access so innovation reaches practitioners in every region.
Q: Are young dentists in the region embracing digital tools?
A: Absolutely! Students and young clinicians are eager to learn and use these skills to grow their careers. Digital dentistry isn’t optional anymore—it’s a gateway to independence, innovation, and better patient care. Education bridges the gap between tech and practice, creating ripple effects across the healthcare system.
Q: Which exocad modules do you use most in your daily work?
A: We use both DentalCAD and exoplan every day. The DentalCAD modules Smile Creator Module, Bite Splint Module, and Virtual Articulator Module are essential for restorative workflows. In exoplan, we use implant planning tools across simple and complex cases—everything from tooth-supported guides to full-arch rehabilitations.
“The mirroring tool in DentalCAD is a lifesaver for anterior esthetics. The mesh editing and alignment tools make collaboration seamless, and virtual extraction with dual scan alignment in exoplan is essential for full-arch planning.”
Q: Do you have favorite exocad features?
A: Definitely. The mirroring tool in DentalCAD is a lifesaver for anterior esthetics. The mesh editing and alignment tools make collaboration seamless, and virtual extraction with dual scan alignment in exoplan is essential for full-arch planning.
And the Smile Creator Module is always a highlight—it brings patients into the process by showing them their future smile in harmony with their face.
Q: Is there a particular exocad feature that feels like a “superpower” in your daily work?
A: Absolutely—custom tooth placement with freeforming and wax-up copy milling in DentalCAD. It gives me artistic and clinical control like never before. It’s not just designing crowns—it’s sculpting functional beauty, tooth by tooth.
Q: Do you have any cases you could share with us?
A: Recently, I used the Smile Creator Module with dynamic jaw motion to design a case where esthetics and function came together beautifully. The patient could see their new smile not just as a picture, but in motion—speaking, laughing, living. That’s the magic of exocad: it turns technology into a story patients can connect with.

This case demonstrates a comprehensive digital smile design workflow using exocad’s Smile Creator Module. The workflow integrates a 3D facial scan, intraoral scan, and virtual jaw motion analysis to produce a fully customized, dynamic 4D wax-up.
Q: What has exocad allowed you to achieve that wouldn’t have been possible otherwise?
A: Freedom. exocad’s open-platform philosophy lets me integrate multiple technologies and customize workflows around each patient. It’s more than software—it’s a philosophy of empowerment.
Q: What would you tell a colleague who’s still hesitant about adopting digital workflows?
A: Start today. The earlier you begin, the more you’ll gain. Digital dentistry is here to stay, and exocad gives you the flexibility to grow at your own pace.
Q: Any fun stories with exocad?
A: One dentist once complained to me that exocad wouldn’t update—only to admit he was using a cracked version! An amusing reminder that shortcuts don’t work—stick with the official software.
Q: Outside of dentistry, what keeps you energized?
A: Spending time with my daughter is my biggest joy. I also love stunt kiting at the Red Sea, swimming, running, and reading. Balance keeps me focused and inspired.
Q: If exocad were an animal, what would it be?
A: A purple panther—fast, slick, and always on target.
